On behalf of the Local Health Authority Commission, the Imperial County Public Health Department applied for funding to implement an Accountable Community for Health (ACH) model in Imperial County. Imperial County was one of 44 jurisdictions that submitted a proposal under the California Accountable Communities for Health Initiative (CACHI) and one of six sites that was ultimately selected for funding.

CACHI is aimed at improving residents' health by aligning and integrating health resources within a community. By aligning activities to address particular health issues, a community's ACHs can achieve meaningful and lasting improvements in personal and population health.  

The Imperial County CACHI team is working to bring key, multi-disciplinary partners together to collectively address asthma--an initial health issue of focus for the Commission, and an impact target prioritized in the Imperial County Community Health Improvement Plan (CHIP).
